 Return Authorization Request Form

If you are dissatisfied with your purchase for any reason, you may return the product(s) in resalable condition within 30 days of the invoice date for a refund. A Return Authorization Code is required. To receive your authorization code please fill out the following form and press the "Request Return" button (located at the bottom of the form.) Once we have verified your request, you will receive a confirmation email containing Return Instructions including any costs, your Return Authorization number (which must be included with your item to return) and an address where you can mail your returned item. You should receive the Return Authorization confirmation email containing your RA number within two business days.
